Supporting documentation

To access Disability Services you will need to provide current supporting documentation from your doctor or specialist. The documentation should include:

You can bring your documentation to your initial appointment with a Disability Adviser or have your practitioner send it as per contact details on the letter requesting information below.

A specialist can include a doctor who is qualified in a specific area of medicine, as well as a psychologist, physiotherapist, occupational therapist or dentist.  The documentation will be used to verify your disability and provide the Disability Adviser with relevant and useful information in assessing appropriate study adjustments or accommodations.
