How do I apply for the National Criminal History Record Check (NCHRC)?

Submitting documents

If you downloaded the application forms from the Clinical Placement Unit (CPU) website access guide, a copy will automatically be sent to the Clinical Placement Unit (CPU) once the Department of Communities and Social Inclusion have processed your application.  Please make sure you also receive a hard copy of your certificate in the mail.  If you do not receive a hard copy from the Department of Communities and Social Inclusion (DCSI) 8 weeks after submitting your application, please contact DCSI to follow up on your certificate.

If you downloaded the application forms from the Department of Communities and Social Inclusion (DCSI) website and not the pre-filled forms from the Clinical Placement Unit website, you will receive a hard copy of your certificate in the mail.  You are required to send an electronic copy of your certificates to CPUOffice@unisa.edu.au

You can check your documents have been received by the Clinical Placement Unit and uploaded onto the system by checking your conditional evidence via myPlacement located through your student portal under ‘my current studies’.
