After downloading the Samstag application form, either:

1. Complete the form directly on your computer, for which you will need Adobe Reader, version 7.0.5 or later. Digitally completed forms – including Sections G and H – MUST be printed and posted to the Samstag office, along with your support material (ie, not forwarded by email).

2. If you do not wish to complete the form on your computer, you should complete the form by hand, with the exception that Section G MUST be typed.

You will need to download and use the appropriate templates below, for your visual documentation in support of your application. See Visual Documentation Guidelines (Section 7 of the Guidelines).
