Please note: The following forms are updated frequently. Please access the forms from this page and do not store a local copy to avoid using outdated forms. All staff forms are for staff access only.

Access

Student Data Access (InfoPath)
Form used to obtain access to the my Teaching section of myUniSA and run online student data reports or to use Student Portal Emulation functionality. Please note Student Portal Emulation Access is only granted to SAS, IT HelpDesk and a few LTU staff, application from other staff will be considered, but are generally denied.

Student Information Systems Access (InfoPath)
Form used to obtain access to Apply Online, Medici and Sorella.

Databee Exams Manager Access (InfoPath)
Form used to obtain access to the Databee Exams Manager website or FileMaker Pro application.

Use this form to add announcements to the Staff and Student Portals.

Credit

Application for Credit (PDF 128KB - download Adobe Reader)
This form has replaced the Application for Credit (Domestic), Application for Credit (International) and Course Substitution form.

Application for Credit - for the Professional Certificate in Academic Practice (PDF 92KB - download Adobe Reader)
This form is to be used to apply credits towards the Professional Certificate in Academic Practice.

Enrolment

Application for Correction of Enrolment (staff use only) (PDF 227KB - download Adobe Reader
Form used to request amendment to enrolment post-census date due to University administrative error or correction.

Study Plan - One Year (PDF 550KB - download Adobe Reader)
Form used when a request has been made to identify and assist a student in confirming their approved credit and remaining program requirements. Less than 12 months of study remain.

Form used when a request has been made to identify and assist a student in confirming their approved credit and remaining program requirements. Over 12 months of study remain.

Fees

Corporate Account Form (PDF 64KB - download Adobe Reader)
This form should be used when an external organisation wishes to pay fees on behalf of two or more students

Internal Payment Request (PDF 32KB - download Adobe Reader)
This form is used to establish an internal payment request where the fees of a student or group of students are to be paid by internal transfer by a Unit/Division of the University

UPS Internal Payment Request (GRC use only) (PDF 39KB - download Adobe Reader)
This form is used to establish an internal payment request where the fees of a student or group of students are to be paid by a UPS scholarship.

Merging IDs

Merging Student Identity Numbers (PDF 25KB - download Adobe Reader)
This form is used for students with two Student Identity Numbers in the Medici Student System

Results

Change of Grade (PDF 105KB - download Adobe Reader)
Form used by academic staff to change student grades

Missing Result or Exchange Result (PDF 120KB - download Adobe Reader)
This form is used to fill blank results or to exchange results. It should not be used for the initial entry of supplementary or deferred results

Student File Retrieval
